What age should my child first see an orthodontist?

The Canadian Association of Orthodontists and the American Association of Orthodontists both recommend a first orthodontic assessment by age 7. Here is why that age, what the visit involves, and what usually happens next.

Why 7

By about 7 the first adult molars have come in behind the baby teeth and the adult front teeth are erupting. That is enough to see how the bite is forming, whether the jaws are growing in proportion, whether there is room for the adult teeth, and whether habits or breathing are affecting growth. It is also early enough that the few problems which are much easier to fix during growth can still be caught: crossbites, underbites, narrow upper jaws, protruding front teeth at injury risk, and teeth heading the wrong way.

What the visit involves

Photos, a scan, a panoramic x-ray if a recent one is not available, a look at the bite, jaws, tonsils and breathing, and a conversation. About 45 minutes, nothing uncomfortable, and complimentary at myORTHODONTIST Maple Ridge. Detail on the first visit.

What usually happens next

Most children are placed on recall: rechecked every 6 to 12 months, at no charge, while the adult teeth come in. A minority start Phase 1 treatment for a specific growth problem. A few have something that needs a dentist or physician first.

Signs not to wait for age 7

• Upper front teeth biting behind the lower (underbite)
• Back teeth biting inside the lower teeth, or the jaw shifting to one side to close
• Thumb sucking or soother use past 4 with the front teeth pushed forward or open
• Snoring, mouth breathing or very restless sleep
• Baby teeth lost very early or very late
• Difficulty chewing or biting, or teeth that meet abnormally

If you missed 7

Nothing is lost for most children. Comprehensive treatment happens at 11 to 14 regardless. For the growth problems listed above, earlier is better, so book when you notice rather than waiting for a milestone.
