Why retainers matter more than braces

Braces or aligners move teeth. Retainers keep them there. Of the two jobs, the second is the one most people fail at, and it is why so many adults in their 30s and 40s are back for a second round of treatment.

Teeth never stop moving

In the year after treatment, the gum fibres that anchor each tooth are still reorganizing and pull the teeth toward their old positions. That is the relapse phase, and it is why retainers are worn full time at first. After that, lifelong change takes over: the lower front teeth crowd gradually in most adults, the arch narrows, and the bite settles with wear. This happens to people who never had braces too. A retainer worn at night is the only thing that holds a finished result against it.

What happens when people stop

Slowly at first. A few months without a retainer and the lower front teeth begin to overlap. A year, and the retainer no longer fits. Five years, and the result is half gone. The teeth do not "settle"; they drift, and the drift does not stop.

Making it a habit

• Retainers live on the nightstand or in the bathroom, next to the toothbrush, never in a drawer.
• Put them in as part of the brushing routine, not as a separate decision.
• Keep the case in your bag for travel; the hotel-room napkin is where retainers go to die.
• If it feels tight one night, that is the retainer working. Keep going.
• If it stops fitting, call before the window closes. A scan on file often means a replacement without a visit.

Fixed wires
